Certificates of insurance naming the city as additionally insured must be emailed, mailed or faxed to 607-274-6587 to the Superintendent's Office. PLEASE NOTE: this form is not for street permit type street closures (construction, tree removal, etc.), it is only for neighborhood and community events.
